I confirm the information above is accurate and I will notify TruIdentity® Spa of any changes to my health, medications or skincare. I understand results vary, that some treatments — including the Perfect Derma™ Peel, dermaplaning, ProCell microchanneling and LED teeth whitening — may cause temporary redness, flaking, dryness or sensitivity, and that I am responsible for following pre-care and aftercare instructions, including daily SPF. I consent to treatment and to before-and-after photos being taken for my chart.
